You can make a self referral on behalf of your child to the agency. You can call your physician, case manager and or social worker. Services MUST be ordered by your physician.

Yes. HSC Home Care can care for your child until 2 months prior to the 22nd birthday.
Business Hours are: 8:30a.m. to 4:30p.m. Monday through Friday. However, an administrator is on call 24 hours a day, 7 days a week.

Yes, consideration is made on a case –by- case basis and will depend on the type of service requested, and qualified staff availability.
Yes, HSC Home Care will partner with you to teach and assist you with your child’s care activities and monitor the progress. HSC Home Care will work the physician and prepare you to care for the child until you can safely care for the child alone.
Progress and goals will be reviewed by the Registered Nurse and/or the Clinical Manager on regular a basis. Once the goals have been met and a safe discharge plan has been determined, the physician will provide the discharge order to the agency staff. The nurse will notify you and again review and answer questions prior to the time and date of the discharge. We will also assist you in identifying other resources that may be needed.
