HSC Home Care Nurses
Nurses at HSC Home Care are highly skilled, caring, and compassionate. The nurses who work at HSC Home Care have made a selective choice to work and provide nursing care and services to infants, children, teenagers and young adults. Being a part of supportive team who view nurses as having role in the continuum of care is highly valued by our nurses.
Providing care in the home and in the communities, is viewed as the frontier of nursing, and is gratefully embraced by HSC Home Care Nurses.
The HSC Home Care nurses realize establishing partnerships for care is essential to care plan development, management and discharge planning.
Our Registered Nurses:
- Skilled observation and assessment
- Carrying physician’s orders and the plan of care
- Teaching patients and families, in order to encourage maximum independence
- Performing procedures and treatments as ordered
- Case management and coordination of care with other disciplines
- Establishes in partnership with the physician, caregiver, family and case manager ,if applicable, safe discharge plans
- Initial and on-going assessment of the patient; their condition and needs
- Update the Plan of Care as the patient’s condition changes
- Instruct parents and caregivers about developmental milestones
- Provide education regarding medications, medication management/supervision, home safety, infection control, equipment safety injury prevention and illness.
Our Licensed Practical Nurses:
- Skilled observation and evaluation
- Works under the supervision of a Registered Nurse
- Carrying out physician’s orders and the plan of care
- Provide medication education and administer medication if needed
- Performing procedures and treatments as ordered
- Teaching patients and families, in order to encourage maximum independence
- Participates in discharge planning
- Monitor and report the patient’s condition and changes
- Instruct parents and caregivers about developmental milestones
- Provide education regarding medications, medication management/supervision, home safety, infection control, equipment safety injury prevention and illness.
